Hospice Referrals & Admissions
A hospice referral can be initiated with the patient’s physician. Physicians, discharge planners, social workers, patients, and families may contact the hospice team to begin discussing eligibility and services.
If the patient elects hospice and is eligible for the program, the hospice team coordinates the arrangements needed for admission and develops a plan of care centered on the patient’s preferences, dignity, comfort, and goals.
